Why do I need to fill out the form when they have all the info already?

For “have you ever had a broken bone” is there a statute of limitations on that, say 35 years?

How about “ever stayed overnight in a hospital” does having your tonsils out when you were 7 really impact anything they are going to do in a random drug screening or DOT recertification?

Or how about staying overnight in the hospital with a sick loved one, do you have to answer yes?

The more forms you have to fill out, the shorter the wait seems. It's mostly just busy work. If they come to get you they'll take the form unfinished and not have you finish.

Ah, but these folks are on to your method. You have to turn the form in before they call you back. lol

don't worry when they can't get LP , or gas the Governor will sing an order letting you drive 24x7 until they no longer need you

Not so sure about that, they went to log book hours for snow removal this year. Between that and the owner not being a fan of the governor, he took a pass this year.

I gotta tell you, after that physical I feel much safer on the road with all the other truckers.

I got one better. Had an MRI on a Thursday. Dr sent me for a ultra sound at the same place the following Tuesday. I wasn't in their system. They wanted to send me a link to fill out the forms in the lobby on my phone. Told her I didn't have it with me. She handed me a laptop !!! Then took my photo ID .and Insurance cards again.

I don’t mind filling out forms but when I have to fill out the same info on multiple forms it irks me a little.

Just got my physical a couple months ago. Im still healthy enough to drink coffee and drive a dump truck. Most of the other drivers cant hardly walk and weigh twice as much as me. But I still have to pay for a physical!
