Anyone have an inspection form they want to share?

I've been drawing up forms and oddly this one is giving me trouble. Does anyone have a form to detail exclusion work needed. One that I could refer to as well as give to customers.

I know this is an older thread but I've been working on my system b/c I tend to overlook a few things. Ive recently been using a 1.50 notebook to take notes on as I talk to the customer about mt findings. During inspection of the structure, I pretty much divide it up into N, S, E, and W, high and low. It is really beginning to improve my efficiency. When I'm finished, I have my list of talking points to use with my customer. Plus, it allows me to 'see' the big picture of what's happening with the structure.
